Mosaic Funeral Group Head Office (3 Rustenburg Road, Krugersdorp West) will administrate the services, but any mosaic branch can be used.

Client is to immediately inform the branch that they are PROSPERCARE members. mosaic Funeral Group will assist the family with all documentation regarding the claim and submit the claim to PROSPERCARE on their behalf.
Mosaic Funeral Group will provide the family with the following documents for each service conducted:

• Original death certificate
• Certified copies of the death certificate
• Certified copies of the deceased’s Identity book
• Certified copy of the BI-1663 (notice of death)
• Certified copy of the BI-14 (Burial order)

Standard resident graves are included in all packages,to the maximum value of Rl 300.00

The bus provided is for local service only for a radius of 50km and is outsourced, should a bus not be available
or if the client does not require a bus, we will give a discount on options 1-4. These packages are available to any PROSPERCARE member or their Family, whether they are covered under the PROSPERCARE plan or not.

Funerals further than 150 kilometers away From place of work that need to
be conducted by mosaic Funeral Group, will be done at a discount of l5% to Prospercare members or their Families.
Any PROSPERCARE member or their Families wanting to erect a tombstone will receive a 10% discount on the service.

Only white coffins to be used For children Funerals. IF parents choose something different, normal prices less 20% discount will apply.
